Holders of Queensland old style paper-based certificates that were issued before March 1992 and that have not been replaced with a plastic certificate are required to notify the Department of Employment and Industrial Relations if they wish to continue to have authority to carry out the work after 1 July 2008.
This web form has been developed to assist holders to give this notice.

Notifiable certificate classes

The web form guides you through each of the following steps. The first 5 steps complete a notification form while the last 2 steps provide information on what and how to submit the form.

1 Enter the personal details of the notifier, including name, date of birth, place of birth, residential and mailing address.
2 Select the certificates currently held that are wished to be kept.
3 Review the translation of authorities that have resulted from the certificates selected.
4 Register the notification. The information is stored in a database held by the Department of Employment and Industrial Relations for referral after the notice has been lodged.
5 Verify notification registration was successful.
6 Generate personalised notification form. Instructions on what must be lodged with the form for it to be complete are provided on the notification form.
7 Mail the final notification with supporting documentation.

The following classes of certificate do not translate to present day authorities and therefore cannot be notified.

Additional information

Not all authorities that are listed on an old style paper-based certificate are still required under workplace health and safety legislation.

Completion of this online process enables a holder to generate a personalised notification form with the new authorities that will be given after the notification has been lodged and processed by the Department of Employment and Industrial Relations, Workplace Health and Safety.
