You are required by law to report certain workplace incidents.
In an emergency

Work health and safety incident notification
An incident is notifiable to WHSQ by law if it arises out of the conduct of a business or undertaking and results in the death, serious injury or serious illness of a person or involves a dangerous incident.
Recording and investigating non-notifiable incidents
Incidents that do not cause serious bodily injuries or illnesses, or are not dangerous events must still be recorded and investigated.
When there is a death in the workplace
A death in the workplace is always investigated to find out what happened and to determine ways to prevent the same thing from happening again.
