Incident:

On 2 May 2006, an employee suffered fracture injuries to his left hand as a result of his hand being crushed in a hydraulic press at the premises of Centwest Engineering and Steel Supplies Pty Ltd, Longreach, Queensland.

Alleged breach:

s.24(1) and s.28(1) Workplace Health and Safety Act 1995

Summary of undertakings:

  • Develop and implement an occupational health and safety management system (OHSMS) consistent with AS/NZ 4804:2001.
  • Engage an OHS consultant to conduct an initial third party audit of the new system within 12 months; followed by a further two audits at 12 monthly intervals.
  • Forward all audit reports to the General Manager WHSQ.
  • Fund, for a period of two (2) years, the NTIS OH&S Unit, “Prepare for Work”, as part of the Rural Vocational Pathways Program for school students.
  • Publish a case study article in the “Safety First” Magazine. 
  • A donation to the Royal Flying Doctor Service.
  • Participate in Flying Doctors Day in 2007 and 2008 via the selling of promotional merchandise.
  • Monitor compliance with this undertaking through a series of third party compliance audits.

The minimum agreed expenditure (including departmental recoverable costs) is $68,335.
