Incident:

On 30 June 2005 a worker was operating a hot glue laminating machine when an incident occurred in which he sustained de-gloving of his right arm. His fingers came in contact with a rotating roller drawing his arm into the machine.

Alleged breach:

s.24(1) and s.28(1) Workplace Health and Safety Act 1995

Summary of undertakings:

  • Develop and implement an occupational health and safety management system (OHSMS).
  • Implement a series of third party audits of the OHSMS, consistent with AS/NZ 4801:2001.
  • Implement the recommendations resulting from the audits of the OHSMS.
  • Engage a consultant to implement programs aimed at building a safety culture and raising safety awareness.
  • Engage a consultant to deliver safety presentations to industry seminars during 2007 and 2008.
  • Appoint and train a Workplace Health and Safety Officer.
  • Fund the participation in a Certificate IV Workplace Health and Safety Course for two persons from identified charitable organisations.
  • Establish and maintain an OHS committee.

The approximate value of the EU (including departmental recoverable costs) is $69,495.
