Incident:

On 21 August 2004 an employee of a concrete pumping contractor was injured when the jib of a crane collided with a concrete placing boom. The collision dislodged a sign attached to the crane. The sign struck the employee causing injuries requiring amputation of the lower part of his left leg.

Alleged breach:

s.24(1) and s.28(3) Workplace Health and Safety Act 1995

Summary of undertakings:

  • Implement a series of third party occupational health and safety management system (OHSMS) audits consistent with AS/NZ 4804:2001.
  • Implement all recommendations unless exempted.
  • Forward all audit reports to the General Manager WHSQ.
  • Provide special training in crane saftey for Watpac’s health and safety officers within the next 12 months.
  • Train approximately 20 Watpac management staff/site managers as Workplace Health and Safety Officers within the next 12 months.
  • Develop a special Anti Collision Controls Industry Flyer which will be made available for distribution by the Construction Safety Managers and Officers Association (CSMOAG).
  • Develop a pocket size manual for crane drivers and project staff.  This manual will be provided to the CSMOAG for the use and benefit of industry.

The approximate value of the EU (including departmental recoverable costs) is $272,630.
