Incident:

On 6 June 2006 a Biniris (Aust) Pty Ltd cleaner was conducting cleaning checks at an abattoir located at Bakers Creek, Mackay. While removing a piece of meat from the evisceration table, the workers coat sleeve became caught in a roller of the table and dragged their arm in and up against a support leg of the conveyor causing a fracture and dislocation to the right forearm.

Alleged breach:

s.24(1) and s.28(1) Workplace Health and Safety Act 1995

Summary of undertakings:

  • A commitment to maintain their occupational health and safety management system (OHSMS) to AS/NZS 4801:2001.
  • Implementation of a series of third party audits of the OHSMS, and implementation of recommendations resulting from those audits.
  • Conduct a range of specified training programs for managers, supervisors and workers on OHS related matters.
  • Active management participation in workplace OHS consultative processes.
  • Development and distribution to staff of OHS related resource material.
  • Donation of an agreed amount to the Queensland Institute of Medical Research.

The minimum agreed expenditure (including departmental recoverable costs) is $72,281.
