Incident: |
On 15 September 2006, an employee suffered fracture injuries to his skull, ribs, vertebra, collar bone and shoulder, a brain haemorrhage, punctured lung and contusions as a result of falling from the top of a bulk fuel tanker truck while refilling the tanker. |
Summary of undertakings:
- Conduct third party audits of the OHS Management System consistent with AS/NZS 4801:2001 with audit recommendations to be implemented;
- Upgrade the standard of guarding to all company fuel gantries;
- All fuel delivery trucks will be equipped with a range of engineered fall protection solutions to eliminate the risk of workers falling;
- Bulk fuel tanks to which deliveries are made, will be equipped with engineered solutions to eliminate risks of workers falling while filling the tanks;
- Install closed circuit television cameras at the fuelling area to provide surveillance of areas where workers normally work alone;
- Promulgation of the circumstances of the incident and subsequent actions to prevent a recurrence, to staff customers and industry;
- Provide staff training with emphasis on safety while refuelling tankers, working at height and identification of hazards and risk assessment;
- Make an ex gratia donation to the Princess Alexandra Hospital Foundation;
- Display and promote safe fuel tank filling systems at two agricultural field days.
This undertaking has a total minimum expenditure of $4,432,085. (Including departmental costs).
