Incident:

On 8 June 2006 a transport driver suffered injuries to his left leg when a load of steel mesh fell while being loaded onto his truck at the OneSteel Reinforcing Pty Ltd Slade Point premises, Mackay, Queensland.

Alleged breach:

s.24(1) and s.28(1) Workplace Health and Safety Act 1995

Summary of undertakings:

  • Conduct a series of third party audits of the company occupational health and safety management system (OHSMS) against AS/NZS 4801:2001 and implement any subsequent audit recommendations.
  • Implement a combination of measures such as the development of policy, training of staff, and delineation of work areas to prevent interaction of materials within workplace exclusion zones.
  • Develop procedures, conduct training and audit compliance with safe load restraint procedures.
  • Develop and implement a driver fatigue management plan.
  • Make available to the transport industry, safe load restraint procedures developed as part of this undertaking.
  • Publish a case study of the incident.
  • Make available to industry a new mesh lifting hook design.
  • Donate an agreed amount to the Mackay Base Hospital.

The minimum agreed expenditure (including departmental recoverable costs) is $91,535.
