Incident:

On 13 January 2007, a fitter employed by QR Limited suffered grievous bodily harm when fingers on his left hand were crushed between the rail line and a moving wheel while working at QR’s Underfloor Wheel Lathe Pit at the Mayne Rail Complex, Bowen Hills, Queensland.

Alleged breach:

s24(1) and s28(1) of the Workplace Health and Safety Act 1995.

Summary of undertakings:

Activities QR Limited will undertake include:

  • implement third party auditing of the Occupational Health and Safety (OHS) Management System to AS/NZS 4801:2001 no less than three times throughout the life of the undertaking with audit recommendations to be implemented
  • provide a copy of the audit reports and actions the obligation holder intends taking in addressing the report recommendations to the Executive Director, Workplace Health and Safety Queensland
  • implement a Zero Harm belief workshop for the underfloor Wheel Lathe Team
  • development of a safety video which can be used in the induction of all new Citytrain Rolling Stock Maintenance staff or as part of refresher training for current staff
  • provide a specific OHS resource to assist the Rolling Stock Maintenance Division
  • establish an annual OHS Network conference/meeting within QR Passenger Services, and
  • community benefit in the form of a $30 000 donation to ‘SPELD’ (Specific Learning Disabilities Association QLD) SPELD is a registered non-profit organisation formed to ensure all Queensland people with specific learning disabilities/difficulties receive equitable educational, social and employment opportunities which will allow for outcomes commensurate with their potential and personal input.

This undertaking has a total minimum expenditure of $198 000 (including recoverable departmental costs).
