Incident: |
On 3 June 2006 at QRs Rolling Stock and Component Services Group site at Redbank, an employee was kneeling on the floor sorting out bolts and nuts for the construction of electrode plates for the tank. He was working in close proximity to two electrode frames each consisting of a number of electrode posts that were standing upright in readiness for steel plates to be bolted to them. A steel plate was being moved into position by an overhead crane when the clamp holding the plate clipped the top of one of the frames causing it to fall onto another frame and both frames to fall onto the employee who sustained a fractured right ankle, fractured nose, laceration to his head and a laceration to his leg. |
