Incident:

On 27 February 2007 a labour hire worker was performing work in the despatch area of Sandvik’s premises. The worker was cutting timber using a docking saw when his left hand came into contact with the saw blade thereby amputating his left index finger and lacerating his thumb.

Alleged breach:

s.24(1) and s.28(1) Workplace Health and Safety Act 1995

Summary of undertakings:

  • Supervisory workplace health and safety training for all five of its managers/supervisors. This three day nationally-recognised training course will be conducted by an appropriately qualified consultant.
  • Sandvik will provide incident investigation training for all five of its managers/supervisors. This training course will be conducted by an external qualified provider.
  • Sandvik will sponsor an industry-wide conference/seminar “The Human Factor in Safety Management”. A session within this seminar will be dedicated to the delivery of a case presentation by a Sandvik representative regarding the circumstances of the incident at the Mackay workshop.
  • Sandvik will provide spinal injury prevention presentations with a road safety theme targeted at the community's future drivers to all Year 11 and 12 students at the Mackay State High School. There are approximately 140 Year 12 and 170 Year 11 pupils at Mackay State High School.
  • Sandvik will implement a worker fitness training program aimed at reducing musculoskeletal injuries, both at work and at home.
  • Sandvik will provide additional workers at the Mackay site with senior first aid training.

The minimum agreed expenditure (including departmental recoverable costs) is $95,474.
