Incident:

On 17 March 2007, a worker was using a machine specifically developed for fitting and removing screw in hose fittings from hydraulic hoses. The worker was holding the rotating hose in gloved hands to remove the hose from the fitting. The gloves became entangled in the hose. The worker sustained injuries to his right forearm.

Alleged breach:

A breach of s24(1) and s28(1) of the Workplace Health and Safety Act 1995, was alleged.

Summary of undertakings:

  • Maintain an OHS Management System in accordance with AS/NZ4804 and undertake a series of third party audits. Implement all recommendations unless exempted.
  • Provide a range of training packages for workers, supervisors and managers to enhance safety and worker wellbeing including health and lifestyle issues.
  • Provide spinal injury prevention presentations to staff.
  • Provide and install a defibrillator and train first aid personnel in its use.
  • Sponsor an Industry Forum in conjunction with the Australian Industry Group. The focus of this forum will be the delivery of information to attendees regarding occupational health and safety issues.
  • Provide spinal injury prevention presentations to all year 11 and year 12 students at a local Rocklea High School.

This undertaking has a total minimum expenditure of $121,000 (Including departmental costs).
