Incident:

On 20 September 2006, a labour hire worker suffered lacerations and fractures when the worker’s arm was caught in a moving conveyor at the company’s Lakes Creek processing facility.

Alleged breach:

s.24(1) and s.28(1) of the Workplace Health and Safety Act 1995

Summary of undertakings:

Activities CMG/Teys Bros. will undertake include:

  • implement third party auditing of the occupational health and safety management system to AS/NZS 4801:2001 no less than three times throughout the life of the undertaking with audit recommendations to be implemented
  • provide a copy of the audit reports and actions the obligation holder intends taking in addressing the report recommendations to the Executive Director, Workplace Health and Safety Queensland
  • implement a conveyor improvement/replacement program
  • develop and implement a Training Guide for Cleaners of plant used in the red meat industry
  • translation of the Training Guide for Cleaners into Portuguese and Vietnamese, and
  • provide a financial donation to assist with the recruitment of a Portuguese or Vietnamese speaking doctor or the purchase of identified medical equipment for the Rockhampton Base Hospital.

This undertaking has an estimated expenditure of $583 000 (including recoverable departmental costs).
