Incident:
On 19 March 2007, a nine year old school student attending a YMCA of Brisbane recreational camping facility suffered grievous bodily harm when he fell approximately six metres from a flying fox adventure ride.

Alleged breach:
s.24(1) and s.28(1) of the Workplace Health and Safety Act 1995
Summary of undertakings:

Activities YMCA of Brisbane will undertake include:

  • implementing third party auditing of the occupational health and safety management system to AS/NZS 4801:2001 no less than three times throughout the life of the undertaking with audit recommendations to be implemented
  • providing a copy of the audit reports and actions the obligation holder intends taking in addressing the report recommendations to the Executive Director, Workplace Health and Safety Queensland
  • creation of a full time Workplace Health and Safety Manager position within the organisation and fund training of the Manager in the Diploma of Occupational Health and Safety
  • appointment and training of a Workplace Health and Safety Officer
  • providing key personal with a range of specified health and safety training
  • development and distribution of a high ropes safety information pack to community and outdoor recreation groups, and
  • convening a two day conference on high ropes safety practices to be attended by community and outdoor recreation groups.

This undertaking has a total minimum expenditure of $52 000 (including recoverable departmental costs).
