Summary


Investigation findings:

Two workers were performing work in an elevating working platform (EWP). Both men were in the bucket and the bucket had been extended to approximately 10 meters. At some time while the bucket was extended, it came crashing to the ground. Both men fell out of the bucket. They were both attached to harnesses which were attached to the bucket.

Investigation findings:

The turret of the EWP had come free of the slewing ring. The turret is attached to the slewing ring by 24 bolts – all the bolts were found on the floor of the truck around the turret, three of the bolts had broken and the remaining bolts all had the tread in tact. It appeared that the bolts had come undone and fallen out.

After the incident Energex:

  • took that particular plant out of service and carried out a check of all slew ring components
  • instigated new procedures.

Obligation holders should consider the requirements of section 27A of the Workplace Health and Safety Act 1995 which outlines obligations relating to risk management. They should also consider the Plant Code of Practice 2005, in particular part 5.13 Inspection and 5.14 Maintenance, Servicing and Cleaning. The Australian Standard AS2550.10-2006 Mobile Elevating Work Platforms.

Court result:

Energex was fined $35,000 with no conviction recorded.
