Matt Thomas Constructions Pty Ltd

Summary

Incident description:

A construction company was building a metal portal frame shed at a residential property. Two workers were working on the roof of the shed at a height of approximately 7.8 metres. One of the workers stepped onto the end of a polycarbonate sheet that was not screwed down and fell through the roof to the concrete floor below, sustaining fatal injuries.

Investigation findings:

No control measures to prevent or arrest a person falling any distance had been implemented.

In this case, there was no fall arrest harness system, fall arrest platform or travel restraint system adopted.

Obligation holders should consider Subdivision 10, Division 3 of part 17 of the Workplace Health and Safety Regulation 1997 which outlines a 'relevant person's obligation for risk of a person falling'.

The company was found guilty and fined $65,000 with no conviction recorded.
