Summary

Incident description:

Two workers removed two temporary braces from a concrete tilt panel before the panel was secured by the steel roof structure. This resulted in a 7.028 m high x 6.605 m wide panel weighing 17.12 tonne falling onto a tip truck. The panel also crushed the adjoining property's fence.

Investigation findings:

Inadequate training and supervision of the two workers and an absence of a safe system of work to identify that the panels were secure prior to the removal of the braces.

Upgraded documentation and improved supervision to ensure proper controls are followed.

Obligation holders should consider the requirements of section 27A of theWorkplace Health and Safety Act 1995 which outlines obligations relating to risk management. They should also consider the requirements of the Tilt-up and Pre-cast Construction Code of Practice 2003, in particular part 13 Work systems and part 14 Panel lifting and erection.

Court result:

The company was placed on an 18 month good behaviour bond with $3,000 recognisance. No conviction was recorded.
