Incident description:
A worker was crushed when he was struck by a bus being driven by another employee.
Investigation findings:
The investigation found that the defendant had not specifically instructed workers in the task of parking buses within the defendant’s compound area and had not formally undertaken any risk assessment into this task. The investigation also found the defendant had relied on the experience of the workers performing this task to do so in a safe manner.
