Summary

Incident description:

A worker suffered a broken wrist when the ladder he was on slipped from under him. He fell approximately 4.5 metres. The worker was untying the top of the extension ladder set up against a two storey domestic house.

WHSQ investigation findings:

The investigation found that the ladder was in an unserviceable condition and did not have non-slip feet. The investigation also found the system used to inspect and maintain plant had not identified the ladder was in an unserviceable condition.

Court result:

The defendant pleaded guilty and was fined $5,000. No conviction was recorded.

Employers must ensure ladders being used by workers are regularly inspected and appropriately maintained. Employers must ensure all workers are supplied with information, instruction and training on the inspection and maintenance of equipment in the workplace.
