Summary

Incident description:

Two workers were injured when approximately 200 square metres of formwork deck they were working under collapsed on top of them when edge supports were removed to redress a decking installation issue.

Investigation findings:

The investigation found that the original formwork design had not been followed and a system of formwork different to the one originally specified had been substituted. It was also found that this system had not been installed according to the formwork documentation.

After the incident the defendant engaged an engineer to examine the existing formwork and followed all recommendations made.

Court result:

The defendant pleaded guilty and was fined $35,000. No conviction was recorded.
