5.0 Application process
Application for accreditation as an Auditor (Self-Insurance) requires the following:
- lodgement of completed application form accompanied by the application fee
- documentary evidence of educational qualifications
- evidence of proof of identity, eg. driver's licence; birth certificate
- passport sized photograph
Fees
An application fee of $639.05 must accompany the application for accreditation. This fee is not refundable in the event that the application fails to meet the accreditation criteria.
An accreditation fee of $639.05 is payable by successful applicants who are issued with a certificate of accreditation and identification card valid for a period of three years.
